Medication Form

If your child needs to take any medication at school, please fill out the Medication Authorization form.  

-All over-the-counter medication must be a child's dose.  A child's weight is not a consideration.

-Only parents are allowed to bring medication, over-the-counter and prescription, to school.

-Medication must be in its original container.  

-At the end of the school year, parents are notified to pick up medication.  Medication that is left will be destroyed.
